Request form is active from 6am – 4pm on the days we are open (Monday- Friday, excluding bank holidays)

Please note that for the following services, patients are able to refer themselves directly without going via a GP

If you require advice, a telephone call or a face-to-face appointment with a GP please fill in the form below.
Note that we can only process forms for patients registered at The Hart Surgery
However,  if you have been registered with us in the past with us and wish to re-register (eg students returning home), please mention that on your form
(If you are unable to fill out the form online our receptionists are happy to help fill it out on your behalf over the phone or at the desk)

This form may be used between 6am – 4pm each weekday that we are open (not bank holidays). If you are able to fill it out during the early half of the morning that would be very much appreciated. If you develop a condition after 4pm and before 6.30pm that cannot wait until we are open again, please call the surgery.  If you feel you need to see or speak to a clinician when we are not open, please call 111.
