Access to Medical Records

The General Data Protection Regulation (“GDPR”) is an EU Regulation which will be directly applicable in the UK on 25 May 2018. It should be read alongside the forthcoming UK Data Protection Act 2018 (“DPA 2018). The GDPR and the DPA 2018 will replace the existing Data Protection Act.

It is your right to see your medical records under the GDPR.

Please see the attached request form which should be completed and brought into the surgery with photo id. We will then process your request and provide you with access to your medical records within 30 calendar days subject to any provisos in Patient Information Leaflet which you can access below.
