Home Visits

Home visits are made by doctors and Gareth Young our paramedic practitioner during surgery hours only. Nurses may also visit to give blood tests, vaccinations and perform basic health checks and reviews. Home visits are for patients who are too ill, infirm or immobile to get to the surgery in person.

If you would like to request a home visit, please fill out our triage form (the earlier in the day the better) which maybe found here.  If you are unable to access the form, our receptionists will be happy to fill out the form on your behalf over the phone. The triage form is not available after 4pm, so please call the surgery after that time or 111 for evening, weekend and bank holiday visits.

We are happy and committed to provide a home visit service to all patients who need it but please remember that several patients can be seen in the practice in the time that it takes to make one home visit so please only request a home visit if you are housebound (or temporarily housebound).

A doctor or nurse may contact you for further information about the nature of your medical condition, to let you know an approximate visit time, to offer advice or to arrange a hospital attendance.

Out of Area Visits

Generally speaking our GPs do not have time in their working day to travel to visit patients who fall outside our catchment area (please refer to practice boundary tab under New Patients section of this site). If you request a visit and live outside our practice boundary we may advise you to telephone the NHS 111 service and they will be able to assess your needs and organise an appropriate clinician to visit you.
