Do you really need to fill out a triage form?

Our triage form should only be used if you need advice from or an appointment with a doctor
(It is usually only active between 6am & 4pm)

Please read the information below before submitting a triage request

• If your query is about medication, please email hart.prescriptions@nhs.net or click here. Your query will be dealt with by our prescription desk or a clinical pharmacist.
• If your query is about booking an appointment with a nurse or healthcare assistant (eg a blood test, smear, dressing, vaccination, contraception, NHS Health check, travel jabs, diabetes, asthma or COPD review), then please either call our treatment room desk between 10am – noon or 2-4pm (via our main line 01491 843200) or email bobicb-ox.hart.trdesk@nhs.net
• If you need administrative help regarding a referral or for non-clinical matters such as insurance forms, shotgun forms, fit to fly letters or medical reports, then please contact our medical secretaries via our administrative form which can be found here.  (They will not be able to help with any clinical matters or book you an appointment).
• For some types of appointment (see below), you do not need to go via your GP but can contact the service directly

We would really appreciate it if you could fill out the triage form yourself but for those who are unable to do so, our receptionists can help fill it out on your behalf over the phone or at the desk.

The form may be used between 6am – 4pm each weekday that we are open (we are not open bank holidays)
If you are able to fill it out during the early half of the morning that would be very much appreciated. If you develop a condition after 4pm and before 6:30 and need GP advice, please call the surgery.
