Do you really need to fill out a triage form?

Our triage form should only be used if you need advice from or an appointment with a doctor
(It is usually only active between 6:30am & 4pm)

• If your query is about medication, please email hart.prescriptions@nhs.net or click here. Your query will be dealt with by our prescription desk or a clinical pharmacist.
• If your query is about booking an appointment with a nurse or healthcare assistant (eg a blood test, smear, dressing, vaccination, contraception, NHS Health check, travel jabs, diabetes, asthma or COPD review), then please either call our treatment room desk between 10am – noon or 2-4pm (via our main line 01491 843200) or email bobicb-ox.hart.trdesk@nhs.net
• If you need administrative help regarding non-clinical matters eg insurance forms, shotgun forms, fit to fly letters, medical resports, then please contact our medical secretaries via our admistrative form which can be found here.  They will not be able to help with any clinical matters.
